Clean root canal systems are essential for successful endodontic treatment. With contributions from leading endodontists from around the world, Dr. Nestor Cohenca here presents the etiology of endodontic disease caused by the endodontic biofilm and all therapies available to predictably disinfect the root canal system, thus increasing successful endodontic outcomes. Disinfection of Root Canal Systems: The Treatment of Apical Periodontitis is an evidence-based manual that describes root canal anatomy, the endodontic biofilm, and the role of disinfection before presenting the most up-to-date methods of irrigation and disinfection. Individual chapters are devoted to each method, such as positive pressure irrigation, apical negative pressure irrigation, sonic activation, photodynamic therapy, laser technology, and ozonization and electrochemical activation. Clinical photographs throughout show proper irrigation and disinfection techniques.

This book serves as a clinical guide to help the practitioner improve endodontic treatment outcomes. It focuses on the various factors affecting the prognosis of endodontic treatments and on their impact on short-term and long-term results. The text incorporates up-to-date knowledge, techniques and treatment protocols. Each chapter has been carefully chosen to address either foundational knowledge or a select aspect of endodontic treatment. The authors analyze the knowledge accumulated from a large number of outcome studies and provide the reader with a critical appraisal indicating the strengths and weaknesses of those studies. This information is then used to make recommendations on how to predict the outcome of the intended treatment. The authors emphasize that the endodontic prognosis is a multifactorial phenomenon, underscoring how various factors, singularly and in combination, influence the treatment outcome. Readers are provided with tools to successfully assess the prognosis of the proposed treatment at the outset and to execute the planned treatment focused on optimal outcome.

Completely updated to keep you absolutely current, this comprehensive reference remains the definitive text for practicing endodontists and general dentists. The ninth edition offers a superb new full-color design and integrates the latest evidence-based research for clinical treatments. It includes coverage of the latest devices and techniques including endodontic instrumentation, obturation, and surgical and retreatment procedures. Providing the latest in treating endodontic infections and pain, and in establishing differential diagnoses, this is a one-stop resource! Records and Legal Responsibilities chapter includes legal forms and descriptions of legal processes, guidelines for record keeping, and updated information on HIPAA/doctor-patient rights and responsibilities. Geriatric Endodontics chapter discusses special considerations and procedures for older patients. Digital Technologies chapter explains how electronic records can reduce paper flow, improve legibility of treatment plans and prescriptions, recall advice given to patients, and record diagnoses; it also shows how technology can be applied to improve patient care. Instruments, Materials and Devices chapter discusses tools and materials used to clean and shape the root canal, including the new MTAD (Endo-Pure®) irrigant, plus appropriate care for instruments, and how to select proper instruments. Information on managing problems/emergencies in endodontic anesthesia explains troubleshooting guidelines for more efficient problem solving, and includes drug interaction tables and guidelines on how to select proper anesthetics. Endodontic Pharmacology chapter outlines pharmacologic options for pretreatment, posttreatment, and intraoperative stages, including current information on COX-2 inhibitors and new drugs on the market. Full-color design offers more detail in clinical photos. New contributors offer expert perspectives and build on the material in previous editions. Endodontic Failures chapter discusses treatment for apical periodontitis. Coverage of biocontaminants and the sterilization of tools and waterlines includes updates on antibacterial procedures and materials, and sterilization pre- and post-treatment, helping practices maintain better infection control. Information on wireless digital radiology describes components and applications, with a pro-and-con comparison to traditional film. Coverage of the connection between apical periodontitis/gingival infection and stroke/heart conditions helps clinicians make better decisions when preparing and treating patients.

This book presents the current state of research on the basic scientific aspects of root canal biofilm biology within a clinically applicable context. Root canal biofilms are complex polymicrobial structures adhering to the root canal surface that are formed by microorganisms invading the pulpal space of teeth, and are associated with persistent root canal infections. Concerted efforts to study root canal biofilms have been made in the past decade, resulting in the publication of observational and experimental studies that detail the morphology and biology of these structures in infected root canals. In addition to confirming that bacteria in root canals do not exist in free-floating planktonic states as previously assumed, this new information on root canal biofilm infections has provided an opportunity to re-evaluate conventional clinical protocols and improve endodontic therapeutic measures.

The purpose of this study was to evaluate systemic inflammation in patients with localized endodontic infection (LI) vs. patients with spreading endodontic infection (SI). 30 patients total, 10 healthy controls, 10 LI patients and 10 SI patients participated in the study. The LI and SI patients presented through emergency visit seeking either root canal treatment or extraction. Before treatment, 10ml blood was drawed by a nurse. Serum from the blood from the three groups was used to evaluate for quantity of C-reactive protein (CRP), serum amyloid A (SAA), tumor necrosis factor alpha (TNF-[alpha]), interleukin 1 alpha (IL-1[alpha]), interleukin 6 (IL-6), and interleukin 1 beta (IL-1[beta]) in each patient using ELISA. Later either root canal or an extraction was performed. Bacterial samples were collected from canals of SI and LI patients using paper points and handfiles and cultured in agar plates for evaluation of amount of bacterial growth. The number of different bacteria morphology were counted and plated in single colonies. The two most prevalent bacteria from each sample was used along with patient serum for Immuno-Checkerboard Hybridization analysis...
