Vulvar disorders are prevalent but often overlooked conditions in the curricula of obstetrics and gynecology, dermatology, and primary care residency training programs. This has led many outpatient clinicians to feel unprepared to evaluate the patient with a vulvar complaint. Often patients with complex, chronic vulvar disorders have seen multiple providers in consultation before they are appropriately diagnosed and treated, leading to frustration amongst patients and providers alike as well as excessive health care costs. The goal of this issue is to present a practical review on vulvar disease for outpatient clinicians to improve patient care. The topics highlighted in this issue represent gaps in knowledge among residents, fellows, providers, and course and conference attendees. The issue begins with a general approach to the evaluation and management of vulvar disorders and then moves on to an article on vulvar pathology that will help clinicians obtain the most useful pathology reads from their biopsies. Also addressed are specific vulvar disorders or presenting findings that are common or challenging. A pelvic floor physical therapy colleague provides information on how this modality can help manage vulvar pain. The issue concludes with an article on hot topics in vulvar disease. In the last several years there has been a growing number of newly trained physicians committed to advancing patient care in the area of vulvar disease, and many of this issue’s authors are part of this group. This issue should serve as a useful resource in the office of any provider who evaluates patients with vulvar complaints.

Tens of millions of people around the world live with chronic pain - many in such severe pain they are disabled by it. The Institute of Medicine estimates that chronic pain costs the U.S. alone $560 to $635 billion a year in direct medical costs and lost productivity. Morphine, an effective painkiller, costs only three cents a dose, yet because of excessive regulation in many countries, it is unavailable to millions of people who need it, even at the end of life. The World Health Organization notes that in addition to the one million end-stage AIDS/HIV patients who can't get morphine and other controlled medications, 5.5 million terminal cancer patients, nearly a million people suffering from accidents or violence, and an incalculable number of people living with chronic illnesses or recovering from surgery don't have access to it, either. Furthermore, women, children, older people, and the poor are disproportionally affected by inadequate pain relief. Physicians know almost nothing about chronic pain, much less how to treat it, for two reasons: medical schools barely teach it and government institutions allot almost nothing to the pain research budget. In The Global Pain Crisis: What Everyone Needs to Know®, renowned health journalist Judy Foreman addresses the most important questions about chronic pain: what is it, whom does it affect most, which pain relief methods in Western and alternative medicine are effective, what are the risks and benefits for opioids and marijuana, and how can the chronic pain crisis be resolved for good? Foreman's book is a wake-up call for a health problem that affects people across the globe, from all walks of life. Written in the classic, easy-to-read and quick reference style of the What Everyone Needs to Know® series, The Global Pain Crisis is a must-read for anyone whose life or work is affected by chronic pain.

This book is about empowerment for chronic pain patients and care providers alike. Every chronic pain condition has a treatable myofascial trigger point component, including fibromyalgia. Many of the localized symptoms now considered as fibromyalgia are actually due to trigger points. The central sensitization of fibromyalgia amplifies symptoms that trigger points cause, and this book teaches care providers and patients how to identify and treat those causes. Chronic myofascial pain due to trigger points can be body-wide, and can cause or maintain fibromyalgia central sensitization. Trigger points can cause and/or maintain or contribute to many types of pain and dysfunction, including numbness and tingling, fibromyalgia, irritable bowel syndrome, plantar fasciitis, osteoarthritis, cognitive dysfunctions and disorientation, impotence, incontinence, loss of voice, pelvic pain, muscle weakness, menstrual pain, TMJ dysfunction, shortness of breath, and many symptoms attributed to old age or "atypical" or psychological sources. Trigger point therapy has been around for decades, but only recently have trigger points been imaged at the Mayo Clinic and National Institutes of Health. Their ubiquity and importance is only now being recognized. Devin Starlanyl is a medically trained chronic myofascial pain and fibromyalgia researcher and educator, as well as a patient with both of these conditions. She has provided chronic pain education and support to thousands of patients and care providers around the world for decades. John Sharkey is a physiologist with more than twenty-seven years of anatomy experience, and the director of a myofascial pain facility. Together they have written a comprehensive reference to trigger point treatment to help patients with fibromyalgia, myofascial pain, and many other conditions. This guide will be useful for all types of doctors, nurses, therapists, bodyworkers, and lay people, facilitating communication between care providers and patients and empowering patients who now struggle with all kinds of misunderstood and unexplained symptoms. Part 1 explains what trigger points are and how they generate symptoms, refer pain and other symptoms to other parts of the body, and create a downward spiral of dysfunction. The authors look at the interconnection between fibromyalgia and myofascial trigger points and their possible causes and symptoms; identify stressors that perpetuate trigger points such as poor posture, poor breathing habits, nutritional inadequacies, lack of sleep, and environmental and psychological factors; and provide a list of over one hundred pain symptoms and their most common corresponding trigger point sources. Part 2 describes the sites of trigger points and their referral patterns within each region of the body, and provides pain relief solutions for fibromyalgia and trigger point patients and others with debilitating symptoms. Pain treatment plans include both self-help remedies for the patient—stretching or postural exercises, self-massage techniques and prevention strategies—as well as diagnostic and treatment hints for care providers. Part 3 offers guidance for both patients and care providers in history taking, examination, and palpation skills, as well as treatment options. It offers a vision for the future that includes early assessment, adequate medical training, prevention of fibromyalgia and osteoarthritis, changes to chronic pain management and possible solutions to the health care crisis, and a healthier version of our middle age and golden years, asserting that patients have a vital role to play in the management of their own health. This text is designed for those clinicians who feel comfortable diagnosing these illnesses and wish to enhance their knowledge base and skill set regarding treatment options. Referrals for pelvic pain are common in urological and gynaecological practice; and may lead to varied diagnoses such as interstitial cystitis/bladder pain syndrome, chronic prostatitis/chronic pelvic pain syndrome, pelvic floor dysfunction, chronic orchialgia, and vulvodynia. To make matters more complex, each of these conditions is frequently associated by co-morbidities. The text is unique in being organized by the multiple and multifaceted therapies that are available, rather than by specific disorders. The text is richly illustrated with multiple diagrams, figures, and tables, making it the “go to” and “how to” reference for patient treatment.

Very few therapeutic agents in clinical medicine have found indication for so many clinical conditions, and in such a short time as did botulinum neurotoxins (Botox and others). Chronic migraine, bladder dysfunction , dystonia, hemifacial spasm , blepharospasm , drooling, excessive sweating and spasticity are all approved by FDA and many other indications are in the near horizon . The aesthetic/cosmetic use of Botox and other BoNTs already has a huge market worldwide. Stroke, Multiple sclerosis, Parkinson’s disease, Cerebral palsy as well as brain and spinal injury are among clinical conditions in which some of patients’ major symptoms can respond to botulinum toxin therapy Several books have been written on the subject of Botox and other neurotoxins for treatment of medical disorders ( including two books by Jabbari both published by Springer 2015 & 2017). However, despite the huge interest and enthusiasm of the public to learn more about Botox and other toxins, there is currently no book in the market on this subject which is specifically designed to inform and educate the public on botulinum toxin therapy. Botulinum Toxin Treatment explains and discusses in simple language the structure and function of botulinum toxin and other neurotoxins as well as the rational for its utility in different disease conditions. Safety, factors affecting efficacy and duration of action, as well as cost and insurance issues are also addressed.

This book (an updated and extended edition) is about mobilizing women and health care policy makers and providers to unite their efforts in a single strategy for fighting cervical cancer worldwide. The objective of this strategy would be to reverse cervical cancer prevalence and mortality rates among all 2.4 billion women at risk and to achieve this goal within 10-15 years of implementation. Cervical Cancer Screening (Pap test, VIA, VILI, or HPV) failed to stop cervical cancer worldwide simply because many countries could not afford developing infrastructure necessary to carry on the global strategy, and because the outreach could not accomplish the targeted 51% of the population at risk. In 2015, there is still 600,000 women getting cervical cancer annually and 300,000 of them die. Every minute one woman gets cervical cancer and every 2 minutes one woman dies from this preventable disease. In 21st Century the Information Technology (IT) Revolution has made substantial impact on medicine enabling remote points-of care, scattered around the world, to be e-connected with experts in distant medical centers and to obtain quality diagnosis and proper guidelines for curative therapy of early stages of cervical cancer. Low frequency of costly interventions needed makes IT-based screening financially and socially beneficial for mass screening. This new Mobile Health technology with the Global Strategy for Fighting Cervical Cancer is subject to elaboration in our book as the new hope when old efforts have failed to stop the world “epidemics” of this grave but preventable disease. The language is adapted for easy reading and understanding by professionals and lay-persons. This book is intended for women at risk for cervical cancer, their health care providers, health insurance companies, government responsible for making health policy and healthcare industry because all of them have special role in the new Global Strategy elaborated in details in this book.
